Consultation Requests
Before 10am Monday to Friday Rather than phoning reception, please submit a consultation request using our online form. All requests are read by an experienced GP in the surgery to decide if we need to offer you an urgent or routine call. Urgency of call back is based on health need, not time of request. You can still call reception if you prefer.
After 10am Monday to Friday If you think that your health concern is so urgent that it can’t wait until the next working day to be assessed, please contact reception by phone or in person. For all other requests for medical help, please submit a consultation request but we may not view this until the next working day.
Weekends and Bank Holidays Please use 111 or 999 if you need urgent help over the weekends or bank holidays
Consultation requests can be submitted between 6.30am and 6.30pm Monday to Friday.
If you are suffering with chest pain that you are concerned could be your heart, severe shortness of breath, have symptoms of stroke or believe that you are seriously unwell, please call 999 and do not use this form to request help.

We are developing a new way of accessing healthcare that we hope will be fairer for all and that those stressful waits on the phone to try to get an appointment will be a thing of the past.
Here are the main changes we are seeking to introduce:
Request help through our website consultation request form.
This is the best way to access help from the surgery. We will be looking out for completed forms and let you know by text how and when we will be in touch to help you. Letting us know what you are hoping for from contact with a doctor will help us work more effectively with you to meet your needs.
Telephone and video consultations.
We will continue to offer telephone or video consultations as a first contact for most health concerns even after the pandemic because we believe that it offers the most convenient, flexible, timely and environmentally friendly access to health care. We know that this doesn’t suit everyone and this can be discussed at the first contact. We are also aware that seeing a doctor or nurse in person is important for a safe assessment of some conditions and can be very reassuring when you are concerned about your health. There will be an opportunity to feedback on the appointment system in the near future.
Access help without having to phone the surgery.
You can also do the following through our online services:
- Request repeat medication (if you don’t already have online access)
- Request a sick note
- Update your details
- Submit information needed for travel appointments, asthma reviews, COPD reviews.
Cancelled Appointments
It is important that you inform the practice if you you are unable to attend your appointment, this will allow that appointment to be offered to another patient. There are 3 ways to do this.

If you are Late
Please attend your appointment on time, if you are late you may not be seen, appointment times may vary during illness and holidays.
Interpreter
If you require an interpreter to attend with you when you see your Doctor please notify the receptionist and this will be arranged.
Private Medical Examinations
Medical examinations for special purposes such as pre-employment, fitness to travel, elderly drivers, fitness to undertake sports etc, will require longer than a routine appointment.
These examinations do not form part of NHS Services, you will be charged for any such examination. Please check the fee chargeable when you require to arrange any such examination.
